1DISCUSS (30 min) • Discuss the anatomical structures of the nose with the class. • Have each student share a trick or memory aid he or she found to remember key anatomical sites in the head and neck. REVIEW (30 min) • Draw on the board a semicircle representing the auricle, with two parallel lines extending from the open side representing the auditory canal. One at a time, have each member of the class come to the board and add an anatomical feature to the drawing and label it. Appropriate Settings: Traditional classroom

Critical Thinking Question

Many patients requiring surgery of the ear have special needs. Some are children, but nearly all have alterations in their hearing. These alterations can affect their perception of the world. How might you influence the environment in the OR to make them as comfortable as possible?

Discussion Guidelines: Students should recognize the need for quiet when the patient enters the OR. Sounds tend to echo in the OR suite, and sensations of the patient are altered, even if he or she is premedicated. Scrubs can help the therapeutic milieu by being adequately prepared, minimizing instrument and supply noise, and keeping conversation minimal and at a low volume.

Critical Thinking Question

All procedures covered in this chapter are performed on the head and neck area. These procedures all involve different approaches in draping and prepping. What other special considerations might apply?

Discussion Guidelines: Students should recognize that the eyes need to be protected at all times. Special effort should be made to keep track of instruments and electrosurgical unit (ESU) pencils by returning them to the Mayo tray.

A tracheostomy is, at times, performed emergently. Review the technique. How is this similar to an emergent laparotomy?

Discussion Guidelines: Students should recognize the importance of visualization and hemostasis in emergent procedures. It is also critical to have the airway on hand during a tracheostomy.

Why should a tracheotomy set be available during thyroidectomy?

Discussion Guidelines: A tracheotomy set should be available during thyroidectomy because of the possible risk of injury to the recurrent laryngeal nerve, which could cause bilateral vocal cord paralysis.
